Upper Canada Family Health Team

The Upper Canada FHT offers a number of services via referrals by doctors in the local area, as well as a number of programs promoting health and disease prevention.

General Medical Care

The family doctors of the Brockville Family Health Organization and Thousand Islands Family Health Organization provide the majority of medical primary care. The Upper Canada FHT is complementing these services with nurse practitioners and chronic disease programs.

Nurse practitioners are registered nurses with additional training and an extended scope of practice so they can provide a full range of health care services to individuals, families and communities. Nurse practitioners provide care to patients of all ages, with a focus on preventing disease, promoting good health and curing illness.

Nurse practitioners can:

  • diagnose and treat illnesses and/or injuries
  • perform physical check ups
  • order and interpret diagnostic tests
  • write/refill prescriptions
  • provide counselling and education
  • provide supportive care through illness
  • provide treatments and/or procedures and
  • make referrals to family physicians, specialists and other health professionals
Mental Health

Provides short term supportive counselling to clients of all ages referred to the program by their physician or nurse practitioner. Many of the clients that are seen by the Mental Health Program are experiencing a situational crisis such as a marriage break-up or the loss of a loved one. The concept behind this program is to bridge the gaps between the existing services provided by community partners and EAP programs.  The program is supported by a consulting psychiatrist.

Gerontology Program
The Gerontology Program assists individuals to remain in their homes and enhance their quality of life by providing education and counselling, interacting with community partners to develop unique service ideas without duplication and providing advocacy on a micro and macro level.
Occupational Therapist
The Occupational Therapist will support patient care and the delivery of programs including chronic disease and geriatric care, by focusing on the identification, provision and management of resources and strategies to improve patient health.
  • Educates and counsels to promote function and independence including health promotion and injury prevention
  • Home Safety Assessment
  • Falls Assessment
  • Activities of Daily Living Assessment
Currently the OT is receiving referrals from the Gerontology Team.
Memory Clinic

For patients who have memory concerns identified by patient, family member or physician / NP.   The patient must be referred to the clinic by a physician or a Nurse Practitioner.

Global Risk Reduction

The Global Risk Reduction Program assists the patient to navigate along the full spectrum of interventions from screening and diagnosis, to treatment, education, collaborative goal- setting and follow up. Based on the client’s needs and promoting self- management through collaborative goal-setting, the client will be assessed for appropriate referral or follow-up to an allied health professional; on an individual basis prior to and /or following group education.

The Global Risk Program consists of:

  • Dietitian
  • Smoking Cessation Nurse
  • Foot care Nurse - Brockville location only
  • Cardiovascular Nurses

Smoking Cessation

The Upper Canada FHT Smoking Cessation program is a FREE educational/support program to help people reduce and/or quit smoking.

Nutritional Manangement

Provides diabetes education, carbohydrate counting, food allergies, weight loss management, cholesterol management, special nutritional needs— all ages

Foot care - Brockville Location

Provides assessment, treatment and health teaching for patients with diabetes.

Brockville location only.

Nurse Navigator - Brockville Location

Works at Brockville General Hospital assisting with continuity of care, education and support of UCFHT-Brockville patients. Participates in discharge planning & reconciles medications from discharge. Ensures follow up appointments are made.
